I consent for my child to attend Daisy Roots (either before, after school and/or holiday club)

I understand that the club has policies and procedures (which are available for reference at the club and on Daisy Root's website), and that there are expectations and obligations relating both to the club and to myself and my child, and I agree to abide by them.

Should my child or any person in my household develop symptoms of Covid- 19 I will follow current government guidance.

I understand that Daisy Roots is a play setting and that whilst my child is there Daisy Roots is legally responsible for him/her.

Once my child arrives at Daisy Roots he/she will be in the care of Daisy Roots until collected and signed out by an authorised person.

If my child is going to be absent, I will notify Daisy Roots before the start of the session. I understand that I will be charged for the booked session.

I will inform St Mary’s School & Daisy Roots of the arrangements made for the drop off or collection of my child and it is my responsibility to relay any changes to these arrangements. (term time)

It is my responsibility to keep the club manager informed of any alterations to the information regarding my child (eg contact details, medical conditions, etc).

I accept that Daisy Roots will use photographs, images and video of my child for promotional material on our flyers, website/social media, leaflets, for medical and special diet identification, newsletters and play observations. I will notify the club in writing if I do not wish my child’s image to be used in this way. Daisy Roots will not sell images to third parties.

My child will be provided with a NUT FREE snack, supper and drinks whilst at the club unless otherwise requested. Dietary requirements will be catered for. (term time)

Whilst Daisy Roots tries to ensure the safety and security of items, I understand that it cannot be held responsible for loss or damage to my child’s property whilst at the club.

I accept that my child may take part in messy activities while at Daisy Roots. I understand that I can provide my child with appropriate clothing to accommodate this if I wish.

Daisy Roots closes at 6:30pm pm during term time and 3:30pm during school holidays. If, due to unforeseen circumstances, I am going to be late, I will contact the manager/deputy as soon as possible.

If I do not collect my child by 6:30pm during term time or 3:30pm during school holidays. I will pay a charge of £10 per quarter of an hour to cover the costs of the staff who are legally required to supervise my child.

If I do not collect my child by 7:00pm during term time or 5:00pm during school holidays, and the club has been unable to reach me or any of my emergency contacts, I understand that Daisy Roots will follow its Uncollected Children Policy and contact Social Care.

If there are any accidents or incidents at Daisy Roots out of school club involving my child, I will be informed.

If my child has an accident at the club, he/she will be treated by a qualified first aider and I will be informed as soon as possible.

If my child needs urgent medical treatment and Daisy Roots out of school club have been unable to contact myself or those listed as emergency contacts on my child’s Registration information, a member of staff from Daisy Roots will sign any consent forms necessary for treatment on my behalf.

I will pay promptly for all booked sessions whether my child attends or not (eg due to illness or holidays)

Running into arrears with payment may result in my child no longer being able to attend the club and will result in additional late payment fees.

Cancellations/ alteration of attendance

Term time

A minimum of one months’ notice is required for cancellation or alteration of my child’s place.

Holiday Club

14 days or more:           100% refund

7-13 days:                     50% refund

Less than 7 days:           We regret that we will be unable to provide a refund

Cancellations made due to illness will, unfortunately, not be refunded.

Please note there is a minimum capacity of 10 children per session required for the Holiday Club to proceed. Should there not be enough children booked, the session will be cancelled and the fees refunded to you.

We will give 72 hours notice in this instance.

A late payment fee may be charged should my payment not be made in advance. Running in to arrears with payment may result in my child no longer being able to attend the club.

I have read the club’s Behaviour Management Policy (available online) and agree to its terms and appreciate that in some circumstances it may be necessary to exclude my child from the club, and I will pay for any missed sessions unless otherwise agreed with the manager.

I understand that aggressive and abusive behaviour towards staff will not be tolerated.

Information held by Daisy Roots regarding my child will be treated as confidential. However, in certain circumstances, for example if there are child protection concerns, I understand that the club has a legal duty to pass certain information on to other agencies, including Police, Social Care and health care professionals.
